We’re excited to share that our MetroWest Care Connection program was recently added to the National Map of Community Care Hubs (CCHs).

This map is published by the Center of Excellence to Align Health and Social Care (COE), part of the Aging and Disability Business Institute at USAging. The map includes dozens of CCHs around the country and can be used by a variety of stakeholders, including policymakers, health systems and other health care organizations and payors, or community-based organizations (CBOs).
