How It Works
When you reach out to the Navigation Line, one of our knowledgeable, compassionate care navigators will return your call within 24 hours, Monday through Friday, to speak with you about mental health and social service concerns you may have for yourself, a loved one, or a person in your care. We’ll take the time needed to listen to your concerns, understand your preferences, and offer personalized service and support options.
Throughout the process, our team will assist you in obtaining direct appointments with our partners or other community resources whenever needed. We also offer the option to coordinate with your primary care physician and/or other service providers to promote healthcare integration and overall collaboration.
Services and Supports
Our care navigators can assist you in linking with a wide range of community resources, including, but not limited to:
Mental health: Outpatient therapy, Psychiatry, Home Based services, and Peer Support
Substance Use and Addiction: Outpatient and Residential services including Medication Assisted Treatment (MAT), Intensive Outpatient Programs, Detoxification, and Peer Support
Social Supports: Housing, Food Security, Employment/Education, Childcare, and Financial Assistance
Healthcare: Insurance enrollment and Primary Care
Eligibility
Anyone of any age located in the following towns is eligible to receive services from the Navigation Line. There is no charge for our service.
