Through our two programs, Behavioral Health Partners of MetroWest (BHPMW) makes it easier for individuals and families to access a continuum of high quality behavioral health, community development, and social services in the greater MetroWest area of Massachusetts and beyond.

MetroWest Care Connection

MetroWest Care Connection is a central hub that assists individuals and families in the MetroWest area in better connecting to services and supports they need to thrive. We partner with healthcare providers, social services, and community stakeholders to enhance the accessibility, coordination, and efficiency of local healthcare and social services. By filling gaps in care and support, we simplify the process for individuals and families, allowing them to focus on their health and well-being without the stress of navigating complex systems.

There is no charge for this service.

Behavioral Health Community Partners

The Behavioral Health Community Partners (BHCP) program, a collaboration among our provider agencies, serves adult MassHealth members who need assistance meeting specific healthcare needs, including physical health, mental health, and substance use.

Our team of compassionate care coordinators works to connect MassHealth members with the services they need. We coordinate care between members and primary care physicians, medical specialists, Accountable Care Organizations (ACOs), and Managed Care Organizations (MCOs), so that everyone’s on the same page.

We also provide supports to address peoples’ social needs, including housing, nutrition, employment, and wellness coaching to achieve goals for better health.
